According to 130.2(B), when energized electrical work is performed as mentioned in Section 130.2(A), an energized electrical work permit is required and must include specific items.

If energized electrical work is needed and can be properly justified, the energized work permit is issued. When issuing the energized electrical work permit, the responsible individuals related to the installation in question weigh in on the final decision to move forward with the energized work. Having several parties involved with the final decision to issue the energized electrical work permit ensures that the energized work is warranted.

Specific items are required to be documented on the energized electrical work permit. The permit must be thorough and include signatures of the authorized responsible parties who agree that the energized work is justified. See below for the elements of an energized electrical work permit.

130.2(B)(2) Elements of Work Permit. The work permit shall include, but not be limited to, the following items:

(1) Description of the circuit and equipment to be worked on including their location

(2) Description of the work to be performed

(3) Justification as to why the work must be performed in an energized condition [see 110.4]

(4) Description of the safe work practices to be employed (see 130.1)

(5) Results of a shock risk assessment [see 130.4(A)]

a. The voltage to which personnel will be exposed

b. The limited approach boundary [see 130.4(E), Table 130.4(D)(a), and Table 130.4(D)(b)]

c. The restricted approach boundary [see 130.4(F), Table 130.4(E)(a), and Table 130.4(E)(b)]

d. Personal and other protective equipment required by this standard to safely perform the assigned task and to protect the worker against any shock hazard [see 130.4(F), 130.7(C)(1) through (C)(16), and 130.7(D)]

(6) Results of the arc flash risk assessment [see 130.5]

a. Available incident energy at the working distance or arc flash PPE category (see 130.5)

b. Personal and other protective equipment (PPE) required by this standard to protect against any arc flash hazard [see 130.5(F), 130.7(C)(1) through (C)(16), Table 130.7(C) (15)(c), and 130.7(D)]

c. Arc flash boundary [see 130.5(E)]

(7) Methods to be used to restrict the access of unqualified persons from the work area [see 130.7(E)]

(8) Evidence of the completion of a job briefing, including discussions of any job-specific hazards [see 110.5(I)]

(9) The energized work approval (authorizing or responsible management, safety officer, or owner, etc.) signature(s)
